Process Details
Member of the Public completes ADA Accommodation form and Medical inquiry
ADA Field Coordinator receives paperwork to approve or deny the request.
ADA Officer initiates an interactive discussion with ADA Coordinator and documents the recommendation to send to the Administrative Head. If the request is approved the ADA Officer creates a person of interest with the requestor information and enter the ADA Accommodation data.
Administrative Official receives email notification about the ADA Accommodation. If the request is denied at the ADA Field Coordinator level, the Administrative Head will take the information received and approve or deny the request. Then send an email notification to the ADA Field Coordinator and the member of the public of the final decision.

Fundamental – high-level

no support, and may refer to a liaison for assistance. |

proficient enough to operate effectively under minimal assistance. Has
understanding of previous process tasks performed and the impact downstream. |

of process, related roles, activities and tasks performed to complete the
